The Contractor is required to submit an invoice for the completed project no later than 30 days <br />from acceptance of the work. Failure to comply could result in the contractor being in breach of <br />the contract and result in dismissal. The City will pay submitted invoices in no less than 30 days <br />from receipt. All invoices shall be itemized by unit price as reflected in pricing schedule and <br />sent to Wayne Masoner, City of Eugene, Public Works Maintenance, at 1820 Roosevelt Blvd., <br />Eugene, OR. 97402. <br />1111W4111 *fl aK_11KQ <br />MATERIALS & WORKMANSHIP <br />The City requires replacing or reconstructing with like materials unless conditions necessitate <br />the use of alternate materials. All alternative materials must be approved by the City prior to <br />installation. Under certain conditions the City may direct the Contractor on the method of repair. <br />All materials and workmanship shall conform to most current City of Eugene /ODOT <br />specifications and APWA Construction Standards and amendments for sidewalks and ADA <br />ramps. <br />Note: These are standard specifications and may require modification for specific job conditions. <br />COMPLETION OF WORK <br />The Contractor shall establish and maintain barriers and warning devises necessary for the <br />safety of the public securing each site during construction. The contractor shall complete <br />restoration of any individual ramp site within five working days from excavation. <br />EROSION PREVENTION <br />Erosion prevention measures shall be incorporated into all work performed within the right -of- <br />way. The intent of those provisions is to ensure that construction related activities prevent or <br />minimize erosion, sedimentation and other storm water related problems. <br />UTILITY LOCATES <br />The Contractor will be responsible to place locate requests two business days prior with the <br />Utility Notification Center prior to excavation or saw cutting in conformance with Oregon Revised <br />statures as adopted by the OUNC in the Standards Manual. The contractor may do so by <br />telephoning the Utility Notification Center at 1- 800 - 332 -2344. General information may be <br />obtained at www.calibeforevoudia.ora. <br />TRAFFIC CONTROL <br />The Contractor shall be responsible for adequate traffic safety control measures in accordance <br />with "City of Eugene Work Zone Traffic Control Standards" and ODOT Temporary Traffic <br />Control Handbook, May 2006, requirements. <br />LANDSCAPING <br />Within 48 hours of the completion of the proposed ramp work, the excavated area behind the <br />ramp will be backfilled with topsoil timilar to Newberg Fine Sandy Loam that is free of debris, <br />roots, stones and materials toxic to plant growth. The loam shall be compacted and held to a <br />grade of Yz" - 3 /+ " below the edges of the sidewalk. <br />The loam shall be seeded with Perennial Ryegrass that is free of noxious weed seed at the <br />manufacture's specified rate enough for the entire surface to be covered. The seed shall be <br />covered with mulch consisting of ground hemlock bark or composted garden mulch in sufficient <br />thickness to completely cover the seed. <br />Page 2 <br />
