Exhibit D <br />SPECIFICATIONS <br />The City of Eugene invites qualified contractors to provide bids for retrofit and construction of <br />sidewalk access ramps within the City's right -of -way to meet American Disabilities Act (ADA) <br />standards. <br />Funding for the Project is being provided in whole or in part from proceeds of a Community <br />Development Block Grant ( "CDBG ") from the U.S. Department of Housing and Urban <br />Development ( "HUD "), and therefore will be subject to HUD regulations. <br />Bidders are required to comply with Executive Order 11246 related to Equal Employment <br />Opportunity. The successful Bidder will be required to use minority and female utilization goals <br />for the percentage of work hours performed in each craft on the Project. Female utilization goal <br />is 2.4 percent and minority utilization goal is 6.9 percent. <br />SCOPE OF WORK <br />The scope of work is to provide for, but not limited to the reconstruction and retrofitting concrete <br />sidewalk access ramps complying with the standards set in the current APWA, Oregon DOT <br />standards and City of Eugene standards and amendments meeting ADA requirements for <br />access ramps and sidewalks. It will also include minor alterations and ordinary repairs or <br />construction of roadways, sidewalk for structural defects, surface drainage and safety issues. <br />The work to be done by the Contractor may include one or more of the following activities; traffic <br />control, excavation, de- watering, erosion control, removal of spoils, backfill, surface restoration, <br />grading and sub grade and paving construction. <br />It shall be the Contractor's full responsibility to have a complete understanding of the type of <br />work to be constructed at each location. The bid will be based on the Contractor's site visit and <br />information proposed in this RFQ. The Contractor shall provide the City with a completed bid <br />and schedule for the ramps. The bids will include the base bid subject to the primary ramp work <br />list identified on the Standard Quote Form and individual prices for the alternate ramps. Bid <br />prices shall include all costs for construction of each ramp, including but not limited to: <br />mobilization, traffic control, de- watering, erosion control, concrete removal, excavation, base <br />crushed rock, finished concrete, asphalt removal and replacement where required, truncated <br />domes, loam backfill, reseeding with bark -o -mulch cover and site cleanup. <br />Upon acceptance and execution of the contract, the Contractor will be given a notice to <br />proceed; the Contractor will then coordinate with City staff in scheduling the work. <br />Once a notice to proceed has been issued and the work has begun, downtime due to equipment <br />failure or staffing problems shall be limited to no more than one work day on any site that <br />construction has begun. <br />The City will designate a staff member to act as the contact person between the City and the <br />Contractor. In addition to any coordination or other contact noted elsewhere in these <br />specifications, the contact designee will act as an inspector during the construction and repair <br />process. The inspector's review of the job site will include but may not be limited to; safety <br />concerns, contract compliance, American Public Works Association (APWA) and City of Eugene <br />/ODOT Specifications and workmanship. <br />Page 1 <br />
