2.B ANNUAL PERMITS FOR MINOR RECURRING ACTIVITIES. <br />Franchised utilities, City-licensed facility operators, and municipal utilities have the option of <br />requesting an annual "blanket" facilities permit for individual service line installations, minor repairs <br />to existing facilities, routine maintenance functions and similar recurring activities. The annual <br />"blanket" facilities permit shall not cover the installation of facilities for expansion or upgrade <br />beyond the activities specifically addressed in the blanket permit. <br />The written request for an annual permit shall be made to the Utility Permit and Inspections <br />Supervisor at 1820 Roosevelt Boulevard, Eugene, OR 97402, by December 31St for the following <br />year, and, at a minimum, shall contain the following: <br />1. Agreement that 2 business-days advance notice shall be provided to the City for all work to <br />be performed under the blanket permit (notification may be provided through use of the <br />Oregon UtilityNotification Center if the notice is for the actual commencement ofwork, not <br />a notification requesting design information, and includes the proposed work location, type <br />of proposed work, starting date, time, and the permittee's contact person's name and <br />telephone number). <br />2. Identification of the types of activities and scope of work (length and diameter sizes of <br />service lines, etc.) requested to be allowed under the annual permit. <br />3. An agreement to submit a monthly report of all blanket permit activities and right-of--way <br />cuts, and of each service installation or repair performed by the permittee's staff or by <br />contractors working for the permittee. <br />4. The name and telephone number of a specific "contact person" who will be responsible for <br />responding to City concerns regarding the permittee's field activities. <br />The request for an annual permit will be reviewed and approved, approved with conditions, or <br />denied. Annual permits are subject to revocation if a facility owner fails to comply with the <br />standards established by the City, these policies and procedures, or standards established by <br />conditions of the permit or other local regulations. <br />Permit fees for annual permits include a processing component and a "per activity" component. The <br />permit processing component is payable upon approval of the annual permit.
