Sample i,etter of Proposed Suspension <br /> This is a due process step. <br /> <br /> ~ ~ Date <br /> Mr. John Brown <br /> Address <br /> Dear John: <br /> Last week you were assigned to attend a presentation by an outside <br /> vendor on a new software package which the office is considering <br /> ~ purchasing.. You were to attend the 3ilay presentation, held at a nearby <br /> (Factual allegation) office building, beginning at 9:00 am and finishing at 4:00 pm. Both <br /> before and after the presentation, you were to report to work at the <br /> ~ ~ office. I have learned that you did, is fact, go on the first day for a <br /> ~ couple of hours but after that you never showed up at the presentation <br /> again. You did come in to work at 8:00 am and indicated that you were <br /> going to the presentation and you did come back to the office after 4:00 <br /> pm and made comments about the software presentation. You indicated <br /> on your time sheets that you were working for the full three days. <br /> When questioned about this, you admitted that you had gone home so <br /> that you could be with your wife since she worked the second shift and <br /> you rarely get to see her. <br /> As you are aware, being dishonest is cause for disciplinary action. <br /> (Employee Handbook) In addition, your indicating on your time sheet <br /> that you were working when you were not, is considered falsification of <br /> (Cite rule violated) your time records and is a violation of state and federal laws as well as <br /> City rules. <br /> This conduct on your part is of sufficient severity for me to .propose that <br /> you be suspended without pay for seven days. <br /> (Sanction proposed) You have the right to respond either orally or in writing to these charges <br /> by (time, date). If you wish to make an oral presentation of your <br /> (Opportunity to respond) position prior to the final decision in your case, you may do so at (time, <br /> place, date). <br /> I will notify you in writing of my final decision based upon your <br /> response.
