He has
been at least 15 minutes late in the mornings for 3 out of 5 days in the last 3 weeks and has been late returning from

i his hinch period on 4 occasions during the same period. Joe said he was having difficulties getting his child ready is
the mornings and that during his lunch hour-he was having to do personal errands for his mother. We discussed how
Joe might alleviate these personal problems and he acknowledged that he needed to take responsibility for being to
work on time.
I warned him that reliability and dependability were important functions of any job and that when he was not here
someone else had to do this work as well as his. He said he didn't realize that others were being impacted by his
actions. I told Joe that I didn't want to see him tardy on a regular basis as he had been in the past 3 weeks and that if
he was, more severe disciplinary action would result.
I told him that this note would be put in his personnel file and I gave him a copy.:
(Signed) (Signed)
Sarah Clear Jce Blow
Supervisor
cc: Jce Blow
HRRS Personnel File
Supervisor's Guide Jannur3', j~
Employee Relations Page 10
