I <br /> I <br /> j is printed on your pay check. <br /> Termination Vacation Pay <br /> If you terminate your employment with the City of Eugene, you will be paid at your current rate of <br /> pay for accrued but unused vacation, up to two times your annual accrual. If you leave before you <br /> are eligible to use vacation, you will forfeit your vacation time. <br /> Witness or Jury Duty <br /> If you are called for jury duty or are subpoenaed as a witness under circumstances beyond your <br /> control and where such duties are in the public interest, you will receive your full salary for the <br /> period of required service. This does not apply when you are involved in personal civil or criminal <br /> proceedings. All monies received as witness fee or as pay for jury duty, except for mileage <br /> ~ allowance, must be signed over to the City, unless such fees were earned on your days off or <br /> ~ during other authorized leave with pay. You are required to report to work when less than a normal <br /> ~ work day is required by jury or witness duties. If you work night or evening shifts, you will be <br /> transferred to day shift during the time you are actually serving as a juror or witness. <br /> I <br /> Leave Without Pay (LWOP) <br /> Should a circumstance arise requiring an absence from work, you may request a leave without pay <br /> in advance. Reasons for LWOP include illness, family illness, educational opportunities, and <br /> parental responsibilities arising from birth or adoption. Leave without pay is normally granted only <br /> after you have exhausted all appropriate paid leave. <br /> i <br /> Request Procedures <br /> Requests for such leaves must be in writing in advance and must establish reasonable justification <br /> for the leave. Except for parental, family medical, or military leave, LWOP will be granted only if <br /> your supervisor determines that your department will not be adversely affected by your absence. <br /> The LWOP period may not exceed 12 months. <br /> ~ Effect on Benefits <br /> Employees on LWOP will not accrue vacation or sick leave. If the leave without pay is for one full <br /> calendar month or more, the City-paid health insurance will discontinue, unless the absence is <br /> covered under the Family Medical Leave Act. Employees may elect to self-pay insurance. <br /> i <br /> City-paid life insurance coverage ceases at the end of the month you exhaust paid leave days, <br /> unless you are unable to work due to disability. <br /> Merit review dates, years of service, probation, and seniority are adjusted for the amount of time an <br /> employee is on leave, except for military leave. Please refer to collective bargaining agreements. <br /> Military Leave <br /> If you have worked for the City for six (6) months, you are entitled to a military leave of absence with <br /> pay not to exceed fifteen (15) calendar days in any one training year for annual active duty for <br /> training as a member of the National Guard, National Guard Reserve, or any reserve component of <br /> the U.S. Armed Forces, or of the U.S. Public Health Service. A training year is the period of time <br /> during which the particular Unit of the National Guard or a reserve component holds 48 consecutive <br /> assemblies for drill and instruction.
