The employee should arrange the leave schedule to minimize <br /> ~ disruption in the workplace, if possible. <br /> Supervisors may require medical certification of the serious health condition. <br /> This request must be in writing. The employee must furnish this certification <br /> within fifteen (15) calendar days from the date of the written request.
