Oregon Parental Leave Law. (ORS 659.360) and Family Leave Law (ORS <br /> 659.560). <br /> 27.2 Employees are entitled to leave under FMLA for the following reasons: <br /> <br /> i <br /> a. birth of a child and to care for the child; <br /> b. placement in the employee's home of a child for adoption or foster care; <br /> c. to care for the employee's seriously ill family member; <br /> <br /> I <br /> j d. a serious health condition of the employee that makes the employee <br /> unable to perform his or her job. <br /> 27.3 Family member means: spouse, son, daughter, parent, parent-in-law, <br /> dependents and any other individuals living within the employee's household. <br /> "Parent" includes persons who have day-to-day responsibilities to care for and <br /> financially support a child or persons who had the responsibility for the <br /> employee when the employee was a child. A biological or legal relationship <br /> is not necessary. <br /> 27.4 LENGTH OF LEAVE <br /> a. Eligible employees may use FMLA leave up to twelve (12) weeks in a <br /> twelve (12) month period and continue to receive their health benefits in <br /> accordance with Article 22. <br /> b. Employees may use sick leave, other personal leave, or may take a leave <br /> of absence without pay, for the qualifying time. <br /> c. Vacation and sick leave accruals will continue while the employee is on <br /> FMLA except during any leave ~of absence coded without pay. <br /> ~ ~ 27.5 REQUEST PROCEDURE <br /> Requests must be made to the employee's supervisor orally or in writing thirty <br /> (30) days before the leave is to begin if the leave is foreseeable. The employee <br /> should arrange the leave schedule to minimize disruption in the workplace, if <br /> possible. The employee may direct any other questions to Human Resources <br /> and Risk Services. <br /> Article 28 <br /> LONG-TERM DISABILITY <br /> 28.1 The City will provide along-term disability benefit that conforms to the <br /> insuring agreements, as set forth in the pre-existing Standard Insurance policy <br /> number 406871, for bargaining unit members disabled due tooff- or on-the job <br /> AFSCME contract - 44 - <br /> <br />
