Unused sick leave is not compensated for upon <br /> termination. <br /> 26.3 An employee requesting time off due to anon-occupational illness or injury or <br /> medical appointment must charge the time to her/his sick leave accrual. If an exception is granted, <br /> the employee may use other available accruals for sick leave purposes. <br /> 26.4 Employees who qualify under the City's Leave Donation Program may receive <br /> time donations in catastrophic circumstances. <br /> 26.5 In addition to qualifying time under state and federal law, employees may use <br /> up to three (3) working days of sick leave per occurrence because of an illness <br /> of a family member when the employee's presence is necessary for the care of <br /> the family member. Family member, for purposes of this provision,'is defined <br /> as the employee's spouse, children, parents, parents-in-law, and the employee's <br /> legal dependents or other individuals living within the employee's personal <br /> household. Exceptions to the three (3) day limit may also be granted by the <br /> City <br /> 26.6 The City may request a doctor's verification of an employee's condition of <br /> health. The purpose of the doctor's verification is to ascertain if the employee <br /> is really unable to report to work or to confirm that the employee is seeking <br /> treatment. When requested, the employee must provide verification <br /> satisfactory to the City's physician. A doctor's verification of illness maybe <br /> required by the City in cases of frequent use of sick leave or when the pattern <br /> of sick leave usage indicates potential abuse of sick leave privileges. In <br /> determining whether such pattern suggests abuse of sick leave, the supervisor <br /> shall consider the number of days per week and hours per day worked by the <br /> employee and particular conditions of the job. Verification may also be <br /> required in any circumstance in which the City determines the employee's <br /> health .status constitutes an obstacle to performing her/his employment <br /> responsibilities. Verification may not be used as a form of discipline. <br /> 26.7 Normal expenses, if any, resulting from verification of illness not to exceed the <br /> usual and customary charges for a doctor's visit will be the responsibility of the <br /> employee. Expenditures in excess of the usual and customary charges will be <br /> paid for by the City, providing the employee informed her/his supervisor and <br /> receives approval for the excess costs prior to incurrence of such liability.
