City of Eugene <br /> Human Resource and Risk Services Policies <br /> 3-838.1 Eligibility (Revised 06/07/99) <br /> Donating Employee: The donating employee must meet the following <br /> criteria: <br /> <br /> i <br /> <br /> j 1. The employee must be a regular or limited duration employee; <br /> 2. The employee must have at least 96 hours of accrued leave (sick <br /> leave, vacation, holiday or comp time) after the donated hours are <br /> removed; part-time employees must have 48 hours accrued leave <br /> after the donated hours are removed. <br /> Receivin Em to ee: The receiving employee must meet the following <br /> <br /> i <br /> criteria: <br /> 1. The employee or member of his or her family has a catastrophic <br /> medical condition that will result in the employee going on leave <br /> without pay status for at least two consecutive calendar weeks; <br /> 2. The employee is a regular or limited duration employee; <br /> 3. The employee must have exhausted all accumulated leave (sick, <br /> vacation, holiday and comp time) <br /> 4. The employee is not receiving workers' compensation benefits, <br /> long term disability benefits or PERS disability retirement benefits. <br /> 5. The employee with a catastrophic medical condition must be <br /> unable to work in his or her regular position or in a modified work <br /> assignment, if made available by the City. <br /> SEE: APPM 3-813.3 and 7-230 <br /> 3-838.2 Procedure (Revised 08/09/02) <br /> Receiving employee: The employee or the employee's representative <br /> requesting a leave donation must complete a Leave Donation Request <br /> Form, available in Human Resource and Risk Services (HRRS), and <br /> should specify the amount of leave being requested. Medical certification <br /> of the illness may be required. Leave beyond that needed will not be <br /> transferred. If the employee is unable to return to work within the <br /> originally anticipated time frame, additional requests for leave donations <br /> may be made, as needed. <br /> Leave donation requests must be reviewed and approved by the receiving <br /> Human Resource and Risk Services Revised: 08/21/02 <br /> <br />
