City of Eugene <br /> Human Resource and Risk Services Policies <br /> employee's Division Manager and Executive Manager to ensure that they <br /> meet the criteria outlined in this policy. Once approved by the <br /> department, the request will be communicated by HRRS to all employees <br /> in the department of the employee making the request.. If insufficient <br /> hours are donated to meet the employee's need, HRRS will communicate <br /> the request to all departments. Every effort shall be made to protect the <br /> confidentiality of medical information throughout this process. <br /> <br /> i <br /> Donating employees: Requests to donate accrued time under this policy <br /> must be submitted on a Leave Donation Form and must be reviewed and <br /> approved by the donating employee's Supervisor to ensure that the <br /> donating employees meet the criteria outlined in this policy. Requests <br /> must indicate the employee for whom the donation is intended, the -total <br /> hours donated and the holiday or vacation account to be adjusted. <br /> Donating employees may not make more than one donation per leave <br /> donation request and may not donate more than 24 hours per leave <br /> donation request; 33.6 hours for Fire Department employees working 56 <br /> hour work weeks. <br /> Once approved, supervisors are responsible for forwarding the Leave <br /> Donation Request Forms to HRRS in a timely manner. Donated hours <br /> are removed from the donating employee's accruals and are transferred <br /> hour for hour to the recipient's sick leave balance each pay period that the <br /> employee is on leave without pay status, so as to maintain the employee s <br /> regular salary and benefits. Donations will be processed in order of the <br /> date received by HRRS. HRRS will notify departmental timekeepers of all <br /> leave donations that will be processed in each pay period. HRRS will also <br /> notify the donating employee when the donation is processed. <br /> The donation, once transferred to the receiving employee, must be made <br /> voluntarily and irrevocably, with the understanding that the donated leave <br /> is lost to the donor forever. All names of all donors will remain <br /> confidential. <br /> 3-838.3 Cash Donations (Revised 10/21/99) <br /> Employees may also make cash donations directly to the receiving <br /> employee to cover other expenses. In addition, employees <br /> may cash out vacation or other personal leave as allowed <br /> under APPM 3-327 or union contracts to make a cash <br /> donation. Employees who are eligible to cash out vacation <br /> or other personal leave should submit a written request to <br /> their supervisor. Requests must be approved by the <br /> _ Executive Manager.
