City of Eugene <br />Human Resource and Risk Services Policies <br />3-700 EMPLOYEE <br />CE. DEFERRED - -: <br />3-710 GROUP HEALTH INSURANCE (Revised 08/09/02) <br />The City provides health insurance for regular employees and their dependents <br />to cover non-occupational injuries and .illnesses. Employees may enroll their <br />domestic partners and the domestic partner's eligible dependents. Contact <br />Human Resource and Risk Services for details. <br />3-711 Eligibility for Coverage <br />3-711.1 Pay Units And Group Health Insurance Coverage (Revised <br />08/09/02) <br />a) Regular Full-Time Employees: All regular full-time employees are <br />eligible for group medical, dental, and vision care insurance for <br />themselves and their dependents. Non-represented employees <br />electing dependent coverage must pay 15% of the cost of <br />dependent coverage. <br />b) Part-Time Employees: Part-time employees have two options: <br />1. City paid medical/dental/vision coverage on the City Health <br />Plan for the employee only. No payroll reduction required. <br />2. Full medical/dental/vision coverage including dependent <br />coverage on either the City Health Plan or the Pacific Source <br />Plan. The employee pays a portion of the premium based <br />on their standard hours designation in the payroll system in <br />accordance with the following schedule. <br />Tier Standard <br />Hours Employee Contribution <br />Tier 32.0 - 40.0 Same amount as for <br />1 Hours/week full-time <br /> employees as defined in <br /> the <br /> 3-711.1 a <br />Human Resource and Risk Services <br />Revised: 01 /23/04 <br />
