City of Eugene <br />Human Resource and Risk Services Policies <br />Tier 24.0 - 31.9 25% of full-time <br />2 Hours/week contributions <br />Tier 20.0 - 23.9 50% of full-time <br />3 Hours/week contributions <br />SEE: Labor Agreements for Represented Employees <br />Note: AFSCME represented part-time employees hired before March 6, <br />1986; and Exempt part-time employees hired before July 1, 1987, receive <br />City paid health insurance. <br />c) IATSE-represented employees are eligible as defined by the <br />current IATSE contract. <br />d) AFSCME represented Limited Duration and Benefitted Temporary <br />Recreation (BTR) employees are eligible as defined by the current AFSCME <br />contract. <br />3-712 Types of Benefits Provided <br />3-712.1 Medical, Dental, and Vision Care (Revised 08/09/02) <br />Regular employees can choose either the City Health Plan (a self-insured <br />plan administered by ODS Health Plan), or Pacific Source Health Plan for <br />their medical insurance coverage. In all cases, the dental and vision <br />coverage is provided by the City Health Plan. Payroll deductions may be <br />required depending on the employee group, the medical plan selected, <br />and/or number of dependents covered. <br />Limited duration and BTR employees will have coverage only through the <br />City Health Plan. <br />SEE: For details of the coverage provided and monthly payroll <br />deductions, see the Summary of Benefits or contact Human Resource <br />and Risk Services. <br />3-712.2 Health Care Coverage For Employees Over Age 65 <br />Medicare-eligible employees will continue to be covered under the <br />City-sponsored plan of their choice.
