i <br /> i <br /> When a City cility is damaged, the permittee shall immediately notify the Inspector. The Inspector <br /> shall submit damage report and a subsequent repair inspection report to the Utility Permit and <br /> Inspections S pervisor. To aid in the updating of the infrastructure condition records, the damage <br /> <br /> ~ repair report hall include a measured location and description of the damage and repair. <br /> In the event t e permittee contests responsibility for the damage to a City facility discovered within <br /> the permittee s work zone, the permittee shall still be held responsible for effecting immediate <br /> repairs. The ermittee should then be directed to submit a billing to the Utility Permit and <br /> Inspections S pervisor showing the exact time and materials expended to effect the repairs. Also, <br /> the permittee should include, in narrative form, an explanation as to the cause of the damage, the <br /> reasons the p rmittee should not be held responsible, and justification for the billing. <br /> Dam ge to Wastewater Collection System - If the damaged facility is a privately owned <br /> buildi g wastewater line, the repairs shall be performed by a contractor, plumber, utility <br /> crew, r others qualified to perform such work. The work shall be performed in accordance <br /> with t e State of Oregon Plumbing Specialty Code (UPC) and inspected by the Wastewater <br /> Inspe for prior to backfill. <br /> All "t ps" into City Wastewater Collection System mains shall be performed by the City. <br /> Whe the damaged facility is a publicly owned wastewater line, the Wastewater Collections <br /> Crew Supervisor shall be immediately notified. The Crew Supervisor shall have the option <br /> of pr viding repairs consultation to the permittee responsible for the damage, directing the <br /> repai performed by the person(s) responsible for the damage, or having the repair performed <br /> by Ci crews at the expense of the permittee responsible for the damage. The Inspector shall <br /> subm t a damage and inspection report to the Utility Permit and Inspections Supervisor who <br /> shall eview and approve a billing submittal to the permittee responsible for repair costs <br /> incu ed by the City. <br /> Dam ge to Stormwater System Components - If the damaged facility is a stormwater <br /> pipe, he repairs shall be performed by a contractor, plumber, utility crew, or others qualified <br /> to pe form such work. The work shall be performed in accordance with City standards and <br /> inspe ted by the Inspector prior to backfill. Repairs involving taps to a stormwater pipe may <br /> be pe formed by qualified contractors or personnel working for the permittee. <br /> Catc basins, inlets, culverts, roadside ditches, and drainage channels damaged by the work <br /> of th permittee shall be restored to as good a condition as existed prior to permit work. <br /> Catc basins shall be cleaned of material or debris resulting from the work. Ditches and <br /> ch els shall be restored to original shape with banks left in stable condition. Rock used <br /> to re tore ditch or channel slopes shall be a minimum 3" and compacted, or as otherwise <br /> speci led by the Inspector.
