A mo thly report of all blanket permit activities and right-of--way cuts shall be submitted, in <br /> an ag eed-upon format, of each service installation or repair performed by the permittee's <br /> staff o by contractors working for the permittee. <br /> 4. The n me and telephone number of a specific "contact person" who will be responsible for <br /> reppo ding to City concerns regarding the permittee's field activities. <br /> The request ran annual permit will be reviewed and approved, approved with conditions, or <br /> denied. A al permits are subject to revocation if a facility owner fails to comply with the <br /> standards est blished by the City or conditions of the permit. <br /> Permit fees fo annual permits include a processing component and a "per activity" component. The <br /> permit proce sing component is payable upon approval of the annual permit. The "per activity" <br /> component is calculated monthly or periodically, based on the number of activities performed under <br /> the annual pe it for which a City response (e.g., site review, inspection) was performed, and billed <br /> to the permitt e. <br /> 2.C RE OCABLE PERMITS FOR PRIVATE FACILITIES <br /> A revocable ermit may be issued to a permittee to provide permission for placement and operation <br /> of private fac lines within the public way which is not authorized by an existing franchise or license. <br /> As with all right-of--way permits, the City reserves the right to "revoke" or cancel the permittee's <br /> permission to use the public way at a later date. This type of permit is typically used for placement <br /> of a private s cture or facility that serves an individual property, site, or business and is not placed <br /> for the provis on of services to others. Detailed questions concerning revocable permits should be <br /> addressed to he Engineering Division personnel at the Permit and Information Center. <br /> Unless other ise specified in a license or franchise agreement, or by the issuance of a temporary <br /> surfacing pe it (TSP), a revocable permit shall be obtained prior to the placement of any private <br /> facility upon over or under the public way. The permittee shall be required to pay all permit fees <br /> and user fee as established. A revocable permit, by itself does not allow for construction to be <br /> performed or for work to occur in the public way. Issuance of a revocable permit does not relieve <br /> the permitt a from obtaining any other permits which may be required.
