The permittee shall notify the Inspector by calling the contact number identified on the permit at least <br /> two (2) business days prior to commencement of work within the public way. "Business day" means <br /> any 24-hour day other than a Saturday, Sunday, or legal holiday. <br /> Placement of the notification through use of the Oregon Utility Notification Center will be <br /> acceptable if the notice is for the actual commencement of work, not a notification requesting design <br /> information, and includes the proposed work location, type of proposed work; starting date, time, <br /> and the permittee's contact person's name and phone number. <br /> Franchised utilities, EWEB, and City-licensed facility operators planning to install a new facility, <br /> or perform a major facility upgrade within the public way, greater than 4001inear feet, shall provide <br /> notice to the City and all other utilities and license holders who are licensed or franchised to provide <br /> services within the project area. The purpose of this notification is to encourage collocation of <br /> facilities and minimize impacts on the public way by providing other utilities and license holders an <br /> opportunity to install facilities in a joint trench or to coordinate work along the same street segment. <br /> <br /> Notification timing and format will be determined by the initiating utility or licensee so long as the <br /> notification purpose and documentation requirements are met and are reasonable. Reasonable <br /> notification shall include at least 10 days advance notice excluding Saturday, Sunday, and legal <br /> holidays. <br /> 2.A.3 Reports Required by the City. <br /> Franchised utilities, EWEB, and City-licensed facility operators must submit a monthly report, in <br /> an agreed-upon format. no later than the tenth of the following month for the following activities: <br /> 1 All active pavement cuts for which permanent restoration has not been completed. <br /> 2 All completed permits, including the completion date. <br /> 2.B ANNUAL PERMITS FOR MINOR RECURRING ACTIVITIES <br /> Franchised utilities, EWEB, and City-licensed facility operators have the option of requesting an <br /> annual "blanket" facilities permit for individual service line installations, minor repairs to existing <br /> facilities, routine maintenance functions and similar recurring activities. The annual "blanket" <br /> facilities permit shall not cover the installation of facilities for expansion or upgrade beyond the <br /> activities specifically addressed in the blanket permit. <br /> The written request for an annual permit shall be made to the Utility Permit and Inspections <br /> Supervisor and, at a minimum, shall contain the following information and stipulations: <br /> 1.
