Please proceed as soon as possible. <br /> Requirements are as follows: <br /> 1) Submit proof of Commercial General Liability, Automobile Liability, Worker's Compensation <br /> and Professional Liability insurance as outlined on the attachment to this letter. Send <br /> insurance certificates to Marge Boutell at the address above.
