The applicant and <br /> contractors collected, chipped and /or removed a total of 730 CY of debris. <br /> The average distance from where the debris was collected to the staging <br /> area of the chipped material was 3 miles. They are participating in the Debris <br /> Pilot Program. <br /> This PW is for the 31 -90 days' work, which is March 9 2014 to May 5 2014 <br /> Total Cost for Work Completed is as follows: <br /> Contract $132,008.67 (see scope notes) <br /> Equipment $19,314.83 the applicant is using their own equipment rates <br /> which are lower than the FEMA rates which were reviewed by the project <br /> specialist <br /> Labor $ 43,444.42 <br /> Disposal Fees $ 541.75 <br /> Total Project Cost $ 195,309.67 <br /> The applicant and contractors took the chips to the following locations <br /> 16th & Jefferson Ally 44.04252 123.10225 <br /> Alton Baker Community 44.05441 123.07564 <br /> Civic Stadium 44.03794 123.09026 <br /> Peterson Barn 44.07016 123.15935 <br /> Washington /Jefferson 44.06126 123.10072 <br /> The applicant affirms via email (attached) that (1) all cut hazard limbs were <br /> at least 2" diameter at the cut; (2) all leaning hazard trees were at least 6" <br /> diameter at 4.5' above the ground; (3) all hangers and leaners posed a <br /> threat to public health and safety if not removed. Addresses of the <br /> approximate locations where this work was performed are given on the <br /> contract billings, which are attached.
