845. Forms, Memos and Billing Information for Street and <br />Security Lighting <br />A. Energy Flat Rate Change Order Information <br />A "flat rate change order" must accompany the street light job from Electric <br />Distribution to Operations. The highlighted portions on the example order on <br />page 32 must be completed by the distribution technician. <br />The following steps, the sample change order on page 32 and the referenced <br />information on page 33 should help answer any questions that may arise. <br />1. Name of agency requesting the street light installation or removal. <br />2. Agency service address (see page 33). <br />3. Agency account number (see page 33). <br />4. Name of agency individual who authorized installation or removal. <br />5. Date the change order is written. a--J of <br />6. Sequence number obtained from screen. By ent;�ing the agency account <br />number and "paging" to the appropriate size code, the sequence number can <br />be obtained. <br />7. Rate code (see page 33). <br />8. Size code (see page 33). <br />9. Change date refers to date of connection or disconnection. This date will <br />be entered by Operations. <br />10. Process code is 23. <br />11.
