<br />EXHIBIT E-6 ATTACHMENT A <br /> <br />STATEMENT OF COMPLIANCE WITH WOMEN <br />AND MINORITY BUSINESS UTILIZATION REQUIREMENTS <br /> <br />Each bidder must comply with women and minority business utilization requirements by making a <br />good faith effort to meet the fair share objectives. The following steps constitute a good faith effort: <br /> <br />1. The bidder solicits qualified minority and women's businesses whenever they are potential <br />sources. <br /> <br />2. If the bidder awards sub-agreements, the bidder shall require the subcontractors to take the <br />steps described above. <br /> <br />3. Submit a completed WOMEN AND MINORITY BUSINESS ENTERPRISES TO BE UTILIZED <br />form with the bid. <br /> <br />The City of Eugene provides additional information to help contractors achieve a good faith effort. <br /> <br />By signing this form, the contractor swears that the steps listed above have been taken to <br />obtain participation by women and minority business enterprises. <br /> <br />The successful bidder may be required to provide a list of women and minority businesses <br />contacted by the bidder but not participating in the contract, along with their reasons for <br />non-participation.
