ARTICLE 5 <br />CONTRACTOR ELIGIBILITY <br />PREQUALIFICATION, QUALIFIED POOLS, RESPONSIBILITY; CONFLICTS OF <br />INTEREST; DEBARMENT <br />P <br />5.1. <br />REQUALIFICATION <br />5.1 .1 UP. <br /> The Solicitation Agent may require prequalification of <br />SE OF REQUALIFICATION <br />potential contractors as a prerequisite to submitting a Bid or Proposal for a particular <br />contract or for listing in a Qualified Pool. Prequalification for construction Services <br />or Public Improvements shall be conducted in accordance with ORS 279C.430. For <br />all other types of contracts, prequalification shall be conducted in accordance with <br />ORS 279B.125. <br />5.1.2PF. <br /> The Purchasing Agent shall approve standard forms for <br />REQUALIFICATION ORMS <br />use in the prequalification of persons to perform construction Services. The form of <br />prequalification for any other Procurement or type of Services may be prepared by <br />the Solicitation Agent. <br />5.1 .3IA. <br /> The Solicitation Documents for prequalification <br />NFORMATION TO PPLICANTS <br />shall provide prospective applicants with information on the standards and process for <br />prequalification, including the deadline, if any, for submitting applications and the <br />time by which prequalification determinations will be made, the forms to be used for <br />submitting prequalification applications, and a statement that no application will be <br />considered unless it conforms to the application requirements described in the <br />Solicitation Documents. <br />5.1 .4 RA. <br /> Upon receipt of a prequalification application, the <br />ESPONSE TO PPLICATION <br />Solicitation Agent shall investigate the applicant as necessary to determine whether <br />the applicant is qualified. In making its determination, the Solicitation Agent shall <br />consider only the Standards of Responsibility applicable to the contract or type of <br />contract for which qualifications are sought. <br />5.1 .5 PD. <br /> The Solicitation Agent shall make a determination <br />REQUALIFICATION ECISION <br />on prequalification of each applicant within thirty (30) days of the deadline for <br />submitting applications, or if no deadline applies, within thirty (30) days of receipt of <br />each application, unless a longer period is provided for in the Solicitation Documents. <br />When prequalification is used for a specific contracting opportunity, the date of <br />determination must be at least fifteen (15) days prior to the Closing of the related <br />contract Solicitation. The Solicitation Agent shall issue a written notice of its <br />decision to qualify or deny qualification to each applicant as soon as the decision is <br />made.
