We hope your experience as a work-study employer has been <br />rewarding and that you wish to continue your off-campus work-study contract with UO. Renewal for the 2008-2009 fiscal <br />year is not automatic and you must re-apply in order to employ work-study students after June 30, 2008. <br />To apply for renewal please submit the following documents: <br />➢ Amendment to Off-Campus Employer Work-Study Agreement (renewal) <br />j ➢ UO Federal Work-Study Renewal Request <br />~S ➢ Updated Student Job Description (one for each position) <br />These documents can be found in step 7 of the Student Work-Study Employment Off-Campus web site at <br />http://baowww.uoregon.edu/OffCampus/ws.htm. Print, complete and return the forms to Kyna Burgett, Office of Student <br />Financial Aid and Scholarships, 1278 University of.Oregon, Eugene, OR 97403-1278. If you cannot download the forms, <br />please contact us and we will fax or mail them to you. <br />After the required documents are received they will be forwarded to the Office of Research Services and Administration for <br />approval. When approved, the Financial Aid Office will send you a confirming e-mail. Your contract will then be extended <br />through June 30, 2009. <br />5/28/2008 <br />
