PROPOSAL SUBMITTAL REQUIREMENTS <br /> <br />Your proposal needs to be submitted in accordance with the submission requirements listed in <br />Section II, Instructions to Offerors, Item 2.0 Submission. Your proposal must contain all of the <br />information requested in the submittal category below. A completeness check will be conducted <br />for each proposal. A complete submittal will make the Offeror a ‘Responsive Offeror’ to be <br />further evaluated for possible inclusion on the Short List as described below. <br /> <br />Submittals <br /> <br />Submitted proposals must include all of the following information: <br /> <br />City of Eugene Standard Proposal form <br />1. - must be signed by a person or persons <br />authorized to represent the firm. <br /> <br />Contact Information <br />2. - Provide the address, telephone and fax number of the nearest office <br />to Eugene, Oregon and the specific name and title of firm’s representative designated as <br />contact. <br /> <br />Experience in Fish Monitoring <br />3. - Provide a summary of your firm’s experience in fisheries <br />biology and monitoring, including any academic training and work experience. Please <br />highlight any monitoring efforts conducted in riverine, side channel, or backwater habitats. <br />Page limit: 3 pages. <br /> <br />Project Methodology <br />4. - Provide a description of the proposed methodology, equipment to <br />be used, and location of monitoring stations. Please present information about why <br />proposed methods and equipment were chosen and describe any biases that may be <br />encountered using proposed methodologies and equipment. Page limit: 3 pages. <br /> <br />Project Schedule <br />5. - Provide a schedule articulating the timing, frequency, and duration of <br />monitoring. Please explain rational behind monitoring schedule (likelihood of sampling <br />success, availability of equipment, etc.). <br /> <br />Project Team Members <br />6. - List all key personnel assigned to the project by name and title. <br />Provide a resume of all persons assigned to the project including the designated project <br />manager. <br /> <br />Conflict of Interest <br />7. - Proposer shall address any potential conflicts of interest with any <br />person or organization affected by actions performed by the firm on behalf of the City of <br />Eugene. Although such involvement will not be an automatic disqualification, the City <br />reserves the right to consider the nature and extent of such work in evaluating the proposal. <br /> <br />References <br />8. – Provide a list of three references that can speak knowledgeably about the <br />consultant’s abilities, experience, and skills in the area of fish biology and monitoring, <br />especially for monitoring efforts conducted in riverine, side channel, or backwater habitats. <br /> <br />Cost Proposal <br />9. - The cost proposal must clearly show the projected budget by major task, <br />with names and billable rates of personnel, plus a general breakdown of anticipated travel, <br />materials and supplies, and other expenditures including general administrative costs. <br /> <br /> <br />
