<br />Safety Plan <br />Prior to start of work consultant shall submit a safety plan that outlines how they will <br />ensure their own safety and that of the public. The plan shall include the name and <br />contact information for an offsite emergency contact, a list the nearest emergency <br />medical facilities, and shall articulate any measures the consultant will take to ensure <br />equipment and methods used are safe. <br /> <br />Permits and Access Permission <br />The City of Eugene will provide necessary federal and state permits for sampling fish. <br /> <br />If access is needed from property other than that owned by the City of Eugene, it is the <br />consultant’s responsibility to obtain permission from the land owner. <br /> <br /> <br />Final Report <br />The primary deliverable is a final report containing the follow elements: <br />a. Summary – a brief overview of project findings; <br />b. Methods - a discussion and photo documentation of the methodology and <br />equipment used, map showing monitoring locations, and any other relevant <br />information; <br />c. Results –tables and charts summarizing monitoring results along with relevant <br />photos (raw data to be provided in an appendix and on a compact disk); and <br />d. Discussion & conclusion – a discussion of results and their relevance to the <br />restored hydrology of the Delta Ponds. <br /> <br /> <br />IV. Project Completion Dates <br /> <br />The fish monitoring shall be completed by May 2012. The Final Report shall be <br />August 15, 2012 <br />completed no later than . <br /> <br /> <br />V. Total Contract Amount <br /> <br />The scope of work and contract amount will be negotiated, but the final contract will not <br />exceed $50,000. <br /> <br /> <br />VI. Minimum Qualifications <br /> <br />The proposer must have demonstrated training, experience, knowledge, and expertise <br />in the field of fish biology and fish monitoring. <br /> <br />VII.
