See http : / /www.ebs.state.or.us /dfcs /id theft.html. To comply with this law, O <br />will only require the Tax ID form: <br />(a) if you are a new Payee not currently listed in the State of Oregon payment system, <br />(b) your mailing address has changed, or <br />(c) your Tax ID number and Entity Status has changed. <br />When required, this form must be faxed and not mailed. The new form with instructions can <br />be found on OWEB's website at: http: / /www.oregon.gov /OWEB /forms.shtml - select Tax ID <br />Form (Word). <br />3. Travel and Other Expenses reimbursement rate sheet (Effective 10- 01 -07) <br />Please read carefully! Any expense not meeting these requirements will be disallowed. <br />4.
