The graph on the right illustrates a five <br />year history of LTD claims. a <br />s <br />4 <br />City of Eugene -Life Insurance Claims z <br />Five Year History <br />3 0 <br />2 <br />City of Eugene -Long Term Disability Claims <br />Five Year History <br />FY03 FY04 FY05 FY06 FY07 <br />Life Insurance <br />No active City employee died in FY07..The graph on <br />the left illustrates a five year history of life insurance <br />claims. <br />EMPLOYEE HEALTH & FITNESS <br />Health Risk Appraisal Program <br />Employees in each department are eligible for a Health Risk Appraisal (HRA) through <br />the Health and Fitness Program every three years. A Personal Wellness Profile (PWP) <br />questionnaire collects information on a variety of health topics including family and <br />personal health history, heart disease and cancer risks, levels of physical activity, eating <br />habits, etc. Since 1991, 886 individual City employees have participated in this program. <br />This group consists of 469 (53%) men, and 417 (47%) women. From these 886 <br />individuals, there have been 1358 HRA's completed over sixteen years which averages <br />out to 85 HRA's per year. <br />Percentage of HRA Participation <br />The goal of the HRA program is to identify Most Recent Year ofEllg/b1/ky <br />health risks, counsel employees to consider ao°~ <br />making. behavioral changes to reduce their <br />personal risks, and ultimately to reduce health 3D°~° 3o°io 3oi <br />care costs for the City of Eugene.
