' 9:: REQUEST FOR PERMIT OR LEASE. ~'i 9.1 Request: Requests for ground leases on the Airport, or for permission to carry on any commercial aeronautical business to serve the public on the Airport, shall be made to the Airport Manager by delivering ali necessary permits and licenses-to the Administration Office in triplicate. The request shall.be in sufficient detail to discern the complete qualifica- tions of the applicant to perform the desired service and shall include the following:, ' < 9.1.1 A written synopsis detailing nature of the proposed activity inci udir~g:'~ 9.1.1.1 The name, address, and telephone number of the.. applicant; 9.1.1.2 'A detailed description of the proposed fixed-b'ase operation that will serve the public including -the proposed date of commencement _Qf~services; 9.1.1.3 The professional qualifications of personnel who will manage and/or operate the proposed fixed-.base operation; 9.1.1.4 Descriptions. and cost estimates of any proposed capital improvements on the fixed-based operation site. 9.1.2 A current financial statement prepared or certified by an independent certified public accountant.
