~ Management Practices Self-Assessment
j ~ 2.1 ~ Operator Qualifications
The agency develops procedures to ensure equipment operators have appropriate
training, licenses and certification.
It is important that the agency maintain a complete record of the equipment operators' licenses to
know who has the legal right and skills to operate equipment. The expiration dates should be
monitored and a routine inspection program should be established to verify that operators have

~ appropriate licenses in their possession. Any suspension of equipment operator licenses must be
reported to supervisory personnel.
Assigned to: Tammy Donahue Initial Review Date: 1.2/11/2002
Completed by: Tammy Donahue Compliance Date:
Amount Complete: Full Compliance
Documentation/Directives:
Supervisor's Guide -Recruitment and Selection
Job Description -License Qualification -Fleet Service Worker
Driving Record Review Form
Fleet Services Policy and Procedure Manual -Repairs
Arial Truck Specifications - 10.6.6
Fleet Board Minutes
Administrative Policies and Procedures Manual
Chapter 13.4.6, 13.4.7, 13.4.10
Forklift Certification List
Monthly CDL Report -This tracking is done in Peoplesoft -available upon request
Agency Compliance:
The Human Resources Division's Recruitment and Selection staff prescreen candidates for the minimum
requirements that are stated in'a job description.
