After making <br /> decisions regarding your health and life insurance benefits, PERS, and deferred compensation, Ip ease <br /> complete and sign the attached Notice of Intent and Notice of Qualifying Event forms and return them to <br /> your supervisor or office manager for attachment to your "Termination PAF." Please call me at x8868 <br /> (682-8868) if you have questions. <br /> Note: If you are retiring under PERS, ask your office manager for the ~~Memo To Retiring <br /> Employees." <br /> Health Insurance <br /> Your City-paid health insurance will cease at the end of the month in which you terminate employment. <br /> You have options of seeking health coverage from a private carrier, under "Portability" (if you are on the <br /> PacificSource Plan), or continuing as a COBRA Continuee through the City of Eugene. <br /> 1. Portabi/ity: If you are currently covered under PacificSource, you will receive "Portability" information <br /> directly from them. Do not confuse this information with COBRA Continuation materials. Portability <br /> coverage provided by PacificSource, has different benefit levels, and an age-based. premium schedule. <br /> Portability is also available at the end of your City of Eugene COBRA eligibility, regardless of your health <br /> plan. <br /> 2. City of Eugene COBRA Continuation: You may continue your health insurance coverage through <br /> the City of Eugene for 18 months, provided you pay the monthly premiums and you are not covered <br /> under another group health plan or entitled to Medicare. You have 60 days after your termination <br /> date to make a decision concerning the COBRA continuation of your insurance coverage. <br /> The City has contracted with BestChoice Administrators to administer the health insurance eligibility and <br /> premium collection for continuees. You will receive information from BestChoice concerning premium <br /> payment options and time frames during the month following separation of employment. <br /> <br />
