Project Review
The policy on voluntary terminations indicates that employees should submit their
resignation in writing to their supervisor, indicating the date of their resignation. City
policy also outlines the process for those employees who are retiring, with specific
information about contacting PERS and. referring employees to the appropriate
contacts in HRRS to complete necessary paperwork.
Supervisors, office managers and PAF (Personnel Action Form) preparers have
access to on-line materials for exiting employees. These memos are specific to the
type of exit, whether it be a resignation, termination or retirement. Most office
managers have developed individualized check lists as an aide for supervisors.
The check list provides step-by-step instructions, including reminding supervisors
to collect keys, uniforms and any other city property issued to the employee. The
Payroll office collects the employee's city identification card when they come to pick
up their final paycheck.
Exit interviews are available on a voluntary basis for any employee leaving the
organization, or moving from one department to another. Key individuals in Human

'i Resources and Risk Services have been trained to conduct these interviews, which
are voluntary and confidential. HRRS looks for trends, and communicates their
observations and feedback to department managers, protecting the identity of the
exiting employee, unless he/she has agreed to have individual feedback given.
Actions to Comply:
Evaluator Suggestions:
Evaluator Requirements:
Evaluator Note:
Project Manager's Signature: Date: ~ L3 6
Agency Director's Signature: Dafe: ~ • ~
APWA Self Assessment-4ff~ Edition # 2.25 'Page 2`of2
