The City requires each bidder to follow specific requirements as <br /> ~ noted below. If the Bidder does not comply with these requirements they will risk the possibility <br /> <br /> i of not receiving a complete solicitation packet and may be deemed anon-responsive bidder. <br /> Please note that if this solicitation is downloaded, it will include all current amendments that <br /> may have been issued since the original bid was posted. The Bidder will not need to download <br /> the current amendments separately. However, the Bidder will be required to complete, sign, <br /> and return a "FAX BACK" form that includes all of their current contact information. By <br /> <br /> ~ submitting this information the Bidder allows the City to send any supplemental information in <br /> addition to the original documents. <br /> Disclaimer: The recipient of any downloaded document from this web site shall be responsible <br /> for completing and returning to the City of Eugene, Purchasing Office the attached "FAX BACK" <br /> form. The recipient shall be responsible for the accuracy of the information on the form <br /> submitted. By doing so the recipient will allow the City of Eugene to send any additional <br /> documentation, including amendments, which may be issued relative to a particular document. <br /> The City takes no responsibility for informing recipients of changes to original documents if the <br /> attached "FAX BACK" form has not been sent to the Purchasing Office. <br /> Any unidentified modifications to the original document issued by the City shall be null and void. <br /> In those instances where modifications are unidentified, the original document issued by the <br /> City of Eugene shall take precedence. The recipient of any downloaded documents is <br /> responsible for clearly identifying any and all changes or modifications to a document upon <br /> submission to the City. <br /> <br />
