2 City of Eugene <br /> Human Resource and Risk Services Policies <br /> ;I <br /> Employees on LWOP will not accrue leave time. <br /> i <br /> Regular full-time employees who are able to work more than a 20 hour per week but less <br /> than a 40 hour week schedule will have the same health insurance options as part-time <br /> employees unless the leave is covered under the Federal Family and Medical Leave Act. <br /> Regular full-time or part-time employees who are unable to work at least 20 hours per <br /> week and have exhausted all accrued leave are not eligible for coverage under the City's <br /> health, life, and long-term disability programs. Health insurance may be continued under <br /> 3-713 or under the Federal Family and Medical Act. Sick leave and vacation hours are not <br /> accrued if the employee works less than 20 hours per week. <br /> i <br /> i 3-872.1 Health Insurance (Revised o8/09/02) <br /> City-paid health insurance benefits will cease at the end of the month in which <br /> the employee has exhausted paid leave days unless the leave is covered under the <br /> Federal Family and Medical Leave Act. Employees can continue their health <br /> coverage while on LWOP by submitting the required premium to HRRS by the <br /> first of each month. <br /> If payment is not received by the 30th of the month, coverage will lapse <br /> retroactive to the first of the month and cannot be reinstated during the remainder <br /> of the LWOP. However, upon return from LWOP, the City will resume the <br /> premium payment, effective the first of the month which the employee returns to a <br /> ~ re lar schedule of at least 20 hours/week. <br /> <br /> ~ ~ ~ <br /> Contact Human Resource and Risk Services for premium and coverage <br /> information. <br /> 3-872.2 Life Insurance (Revised 08/09/02) <br /> City-paid life insurance coverage ceases at the end of the month the employee <br /> exhausts paid leave days, unless the employee is unable to work due to disability. <br /> The City will continue to pay the life insurance premium for employees on <br /> medical leave of absence providing they have filed for long term disability. <br /> Contact HRRS for information. <br /> Employees who purchase supplemental life insurance may continue to pay the <br /> monthly premiums to HRRS while they are on LWOP. <br /> Human Resource and Risk Services Revised: 09/16/02 <br /> _ _ _ _ <br /> <br />
