Central Services <br />Human Resource <br />~~ t ; And Risk Services <br />City of Eugene <br />777 Pearl Street, Room 101 <br />MEMORANDUM (541> 6820- 061n 97401 <br />(541) 682-6831 FAX <br />www.ci.eugene.or.us <br />Date: May 24, 2004 <br />To: City of Eugene Non-Represented Part-Time Regular Employees <br />From: HRRS Benefits Program Staff <br />Subject: Part-Time Employee Health Insurance Options for FY2005 <br />Non-represented regular part-time employees have the following health insurance options during the <br />Health Insurance Open Enrollment Period: <br />No-Cost Option: Non-represented City of Eugene employees may "opt-out" of City-provided <br />health insurance coverage with proof of other coverage. Completion of an Opt-Out <br />Notification form is necessary. <br />2. Low-Cost Employee-only coverage (Medical/Dental/Vision: This option has the <br />following features: <br />^ Full medical/dental/vision coverage for you as the employee through the City Health <br />Plan (administered by ODS Health Plans); <br />^ No dependent coverage; and <br />^ Cost to the part-time employee is the same cost as a full-time employee pays for City <br />Health Plan Individual coverage. <br />3. Medical, dental, and vision coverage for you and your dependents under the City Health <br />Plan or PacificSource. The attached Comparison of Benefits highlights the coverage provided <br />under these plans. <br />Part-time Non-represented employees opting to cover dependents under the City Health Plan <br />or opting for full coverage for themselves (with or without dependents) under PacificSource <br />are required to contribute a portion of the insurance premium in most cases. Cost is based <br />on their standard hours designated in the payroll system. The enclosed grid shows the <br />deduction amounts per pay period (first two pay periods in each month) effective July 1, <br />2004. Deductions are based on the work schedule tiers. <br />NOTE: Employees who began their part-time Non-represented Exempt position prior to Ju/y i, 1987, are <br />"grand-fathered," and have payroll deductions the same as full-time Non-represented regular employees. <br />If you have questions, contact Benefits Staff in Human Resource & Risk Services (HRRS) at x8868 or <br />x5924. <br />(Continued on Reverse) <br />
