' FY2005 Health Plan Open Enrollment (Non-Represented Employees) <br />May 24, 2004 <br />Page 2 <br />All Non-represented employees are required to pay the payroll deductions for health coverage, <br />including those employees married to other City employees where each is covered under the other's <br />health plan. <br />However, Non-represented City of Eugene employees may °opt-out" of City-provided health insurance <br />(medical, dental, and vision) coverage, with proof of other coverage. The proof (in the form of a copy <br />of wallet ID from another insurance carrier, or similar documentation) must be attached to the °Opt- <br />out" form. Contact Benefits Program staff for a copy of the City of Eugene Non-Represented <br />Employee Health Insurance Opt-Out Notification form. <br />Part-time Non-represented employees are required to pay a portion of the premium for medical, <br />dental, & vision coverage prorated to their work schedule. Part-time employees will receive additional <br />information on payroll deductions and coverage options. <br />Benefit Changes Effective 7uly 1, 2004 for Non-Represented Employees <br />Citv Health Plan (administered by ODS Health Plans): <br />• No Changes in Benefits <br />• An updated ODS Health Plans preferred prescription drug list is included with the Comparison of <br />Benefits. <br />PacificSource Health Plans: <br />• No Changes. <br />Comparison of Benefits <br />The enclosed Comparison of Benefits highlights the coverage provided under the plans and is <br />designed to help you compare and evaluate the plans before you enroll. The Comparison of Benefits <br />summarizes the general benefits under each plan and does not provide a full description of benefits. <br />Information sessions will not be held this year, as there are no benefit changes to the plans.
