No-Cost Emnlovee-only coverage (Medical/Dental/Vision): This option has the <br />following features: <br />^ Full medical/dental/vision coverage for you as the employee through the City Health <br />Plan (administered by ODS Health Plans); <br />^ No dependent coverage; and <br />^ No cost to employee. <br />-- OR - <br />2. Medical, dental, and vision coverage for you and your dependents under the City Health <br />Plan or PacificSource. The attached Comparison of Benefits highlights the coverage provided <br />under these plans. <br />Part-time AFSCME-represented employees opting to cover dependents under the City Health <br />Plan or opting for full coverage for themselves (with or without dependents) under <br />PacificSource are required to contribute a portion of the insurance premium in most cases. <br />Cost is based on their standard hours designated in the payroll system. The enclosed grid <br />shows the deduction amounts per pay period (first two pay periods in each month) effective <br />July 1, 2004. Deductions are based on the work schedule tiers. <br />NOTE: Employees who began their part-time AFSCME-represented Exempt position prior to March 6, 1986, <br />are "grand-fathered," and have payroll deductions the same as full-time AFSCME-represented regular <br />employees. <br />If you have questions, contact Benefits Staff in Human Resource & Risk Services (HRRS) at x8868 or <br />x5924. <br />(Continued on Reverse) <br />
