FY2005 Health Plan Open Enrollment (AFSCME-Represented Employees) <br />May 24, 2004 <br />Benefit Changes Effective ]uly 1, 2004 for AFSCME-Represented Employees <br />City Health Plan. (administered by ODS Health Plans: <br />• No Changes in Benefits <br />• An updated ODS Health Plans preferred prescription drug list is included with the Comparison of <br />Benefits. <br />PacificSource Health Plans: <br />• No Changes. <br />There are no other benefit changes for the AFSCME group at this time. You will be notified of any <br />benefit changes resulting from current AFSCME contract negotiations. A separate open enrollment will <br />be scheduled if necessary. <br />Comparison of Benefits <br />The enclosed Comparison of Benefits highlights the coverage provided under the plans and is <br />designed to help you compare and evaluate the plans before you enroll. The Comparison of Benefits <br />summarizes the general benefits under each plan and does not provide a full description of benefits. <br />Information sessions will not be held this year, as there are no benefit changes to the plans.
