If you enter military service for extended periods of <br />active duty, leave without pay is allowed in accordance with Oregon State laws. See APPM 3-840 <br />for more information. <br />Health Benefits (Medical, Dental, and Vision) <br />The City provides health insurance for regular employees and their dependents to cover non- <br />occupational injuries and illnesses. The effective date of coverage is the first day of the month <br />following your date of hire. <br />Coverage <br />Medical, dental, and vision coverage is available. Payroll deductions may be required depending <br />on the employee group, the medical plan selected, and/or number of dependents covered. For <br />details of the coverage provided and monthly payroll deductions, see the Summary of Benefits on <br />the HRRS Intranet site (http://ceonline/HRRS/RISK/Benefits/Hlthlns.htm) or contact HRRS Benefits <br />Program. <br />During health insurance open enrollment, you can reconsider your choice of medical coverage. <br />Open enrollment, usually held in May or June, is the only time during the year you can change from <br />one medical plan to another. <br />Termination <br />Your health coverage will cease at the end of the month in which you terminate employment. You <br />may continue insurance coverage on aself-pay basis. Contact HRRS for details. At the end of the <br />continuation coverage, you may elect a portability plan if offered by the carrier. <br />Life Insurance <br />The City of Eugene pays the cost of life insurance for all regular full- and part-time employees. <br />Benefit maximums vary by bargaining unit. See HRRS Intranet site <br />(http://ceonline/HRRS/RISK/Benefits/Lifelns.htm) or contact HRRS for more information. <br />Accidental Death or Dismemberment <br />In addition to the above coverage, if you die as a result of an accident, your beneficiary will receive <br />an additional amount (maximums vary by employee group). A lump sum benefit is paid for loss of a <br />limb or an eye as the result of an accident. The amount varies depending on the loss. <br />Supplemental Life Policy <br />Regular employees are eligible to purchase additional life insurance coverage for themselves or for <br />their dependents. See HRRS Intranet site (http://ceonline/HRRS/RISK/Benefits/Lifelns.htm) or <br />contact HRRS for information on rates. <br />Long-Term Disability <br />All regular full-time and part-time who are regularly scheduled to work at least 20 hours per week, <br />are covered by a long-term disability policy. <br />Employees eligible for long-term disability benefits will receive a percent of their scheduled salary to <br />a monthly maximum benefit. This varies by employee group. Long-term disability benefits are for a <br />maximum of 2 years, except in cases of total and permanent disability.
