Entitlement to parental leave expires at the end of the 12-month period beginning on the child's <br />date of birth or on the date of physical custody of a foster child or newly adopted child. <br />When two family members both work for the City of Eugene they may take Family Medical and <br />Parental Leave at the same time with supervisory approval. <br />Request Procedure <br />Family Medical Leave: Oral or written notice must be provided 30 days before the leave is to begin <br />if the leave is foreseeable. The employee should arrange the leave schedule to minimize disruption <br />in the workplace, if possible. <br />Supervisors may require medical certification of the serious health condition. This request must be <br />in writing. The employee must furnish this certification within fifteen (15) calendar days from the <br />date of the written request. Medical certification forms are available from HRRS. <br />Parental Leave: Oral or written notice must be provided 30 days in advance of the anticipated date <br />of birth or placement in the employees' home of a child for adoption or foster care, except in cases <br />of premature birth, unexpected adoption or unexpected foster placement. <br />All requests should be forwarded to HRRS. <br />Effect on Benefits <br />Health insurance benefits will be maintained while you are on an FMLA family medical or parental <br />leave under the same conditions as coverage would have been if you had been continuously <br />employed during the entire leave period. <br />Reinstatement <br />When you return from leave you will be reinstated to the same position you held when the leave <br />began, orto an equivalent position with equivalent benefits, pay, and otherterms and conditions of <br />employment if your former job has been eliminated. Seniority does not accrue during any portion of <br />the leave which is without pay. <br />Compassionate Leave <br />In the event of a death in your immediate family (as defined by your applicable bargaining <br />agreement or the APPM), your supervisor may grant time off with pay for you to make funeral <br />arrangements and to attend the funeral. A maximum of five calendar days may be granted if <br />warranted by the situation (i.e., distance to travel, necessity to make arrangements, etc.). <br />Compassionate leave is not charged against sick leave. If you serve as a pallbearer or attend the <br />funeral of a co-worker, compassionate leave of up to four hours may be granted. <br />Holidays <br />The City observes the following Holidays: <br />New Year's Day, January 1 <br />Martin Luther King Jr. Day, third Monday in January <br />Memorial Day, last Monday in May <br />Independence Day, July 4 <br />Page 21-February 2004 <br />
