City of Eugene <br /> Human Resource and Risk Services Policies <br /> manner. <br /> 3-115.2 Procedures for Potential or Current Employees (Revised os/14/o4) <br /> 1. All requests for reasonable accommodation from a qualified applicant being <br /> <br /> i offered a position or a qualified employee with a disability should be directed <br /> to the hiring or employee's supervisor. Any supervisor receiving a request <br /> should contact Human Resources for guidance and assistance in responding <br /> to the request. <br /> 2. The supervisor will meet with the employee requesting the accommodation <br /> within five working days of receiving the request to discuss the limitations <br /> resulting from the disability, their impact on the essential functions of the job <br /> and potential accommodations that could address those limitations. <br /> 3. The employee will be required to provide a written statement from his/her <br /> physician to verify he/she has a qualifying disability and the precise <br /> limitations resulting from the disability, as well as recommended <br /> accommodations, within ten working days of being requested to do so. The <br /> physician maybe asked for additional information or supporting documents <br /> needed to assist the City in determining whether the employee has a <br /> qualifying disability as defined by the law and, if so, what reasonable <br /> accommodations might be made. Additionally, the City may require the <br /> individual to be examined by a medical expert selected by the City or to <br /> participate in other appropriate assessments to assist the City in determining <br /> the appropriate course of action. <br /> 4. The supervisor, in consultation with Human Resources, will determine the <br /> feasibility of the accommodation requested, or other alternative reasonable <br /> accommodations, in light of all the circumstances. <br /> 5. The supervisor will notify the employee of the City's decision regarding <br /> accommodation within ten working days of receiving the information from <br /> the employee's physician. If additional time is needed to make the <br /> determination or to provide the reasonable accommodation, the supervisor <br /> will notify the employee of the date by which they will be informed. <br /> 6. The City will maintain the employee's medical information in a confidential <br /> manner and in a separate, confidential file . <br /> Human Resource and Risk Services Revised: 05/14/04 <br /> <br />
