~ <br /> WHAT ARE THE APPLICATION REQUIREMENTS FOR A SMALL GRANT? <br /> i <br /> Applicants requesting $100,000 or less in funding from EPA must provide the following: <br /> 1. Application for Federal Assistance (SF-424), the official form required for all Federal j <br /> grants that requests basic information about the grantee and the proposed project. The ~ <br /> applicant should submit the original application and one copy (unless otherwise <br /> re uired b rant re ulations si ned b a rson dul authorized by the governing <br /> q Y g g g Y Pe Y <br /> board of the applicant. <br /> 2.. The Federal Standard Form (SF-424A), which provides information on your budget. <br /> For the purposes of the Small Grants policy, complete only Item Number 6 of <br /> SF-424-A dividing your budget into categories such as personnel, salaries/fringe <br /> benefits, travel, equipment, supplies, contractual, and other costs. <br /> ' 3. Narrative/workplan of the proposal, in most cases, should not exceed five pages in ~ <br /> length. A narrative/workplan is one of the most important aspects of your application <br /> for a Small Grant and will be used as the prunary basis for selection. <br /> The workplan serves as the performance commitment and should describe as accurately <br /> and concisely as possible: <br /> <br /> I <br /> 1) a. summary of specific objectives, expected outcomes, outputs and <br /> deliverables; and <br /> 2) a discussion of the budget and how the budget relates to the o ~ectives, <br /> outcomes and deliverables: I , <br /> NOTE: If <br /> you. ar_e purchasing any equipment which costs $5,000 or more, each <br /> piece urcd il's cost must be specifically listed in the workplan. <br /> The workplan may include a chronological schedule of accomplishments, progress, and <br /> milestones that are anticipated over the project's duration.::: <br /> 4.---- The necessary signed-foizns-such as the_Certificati9n_IZegardmg Debarmentz <br /> -Suspension, and Other Responsib~ity Matters.
