' RenT rtin~and Record Keeping Requirements . <br /> 1. .The City of Eugene will submit two reports on the progress of the project supported by this <br /> Grant. The report forms will be included with the first installment payment by APA to the City <br /> . of Eugene. The first report will be due to APA no later than February 13, 2003. The second <br /> . and final report must be submitted upon completion of the project. In addition to the <br /> information requested in the forthcoming report guidelines, the reports shauld include the . <br /> following: <br /> . ~ a. Copies of educational materials, brochures, reports and any other representative <br /> ,I materials relating to the project; (interim and final report) <br /> b. Copies of any media coverage generated by the project; (interim and final report) <br /> c. Project accounting information. Information should include a comparison of the <br /> actual expenses to the approved line item budget and an explanation for any <br /> - overages and/or unspent funds. (interim and final report) ` <br /> r <br /> d. The City of Eugene's most recent audited financial statements. (final report) <br /> Publicity and Acknowledg_en_t <br /> r <br /> Acknowledgment for this Grant should include: <br /> <br /> i <br /> 1. A general acknowledgment of APA among the City of Eugene's list of annual funders in all <br /> . .appropriate programs, publications, web pages, and public announcements for. the Grant period. <br /> 2. Use of the "American Planning Association" in any public documents pertaining to this Grant. <br /> Prominence of acknowledgment on all printed materials should be commensurate with the <br /> level of funding relative to other funding sources. <br /> <br /> r 3. APA reserves the right to include information relating to the Grant in APA's periodic reports, <br /> newsletters or news releases or in other materials issued by or on behalf of APA. <br /> 4. APA will work with the City of Eugene to develop a press release, which must conform to the <br /> following guidelines: . <br /> a. APA should be named in the first pazagraph: . <br /> b. Provision should be made in the release for a quote from a representative of APA. <br /> c. The following description of The City Parks Forum should be included in the . <br /> announcement: <br /> The City Parks Forum S^~is a program of the American Planning Association, whose . <br /> mission is. to advance the art and science of urban, rural and regional land-use <br /> planning.
