I have prepared this proposal using the Oregon Department <br /> of Transportation (ODOT) Statewide Permits Template that you provided me. Because the proposed <br /> project is planned for construction in 2010, the goal into have all documents described below delivered to <br /> the City of Eugene (City) by May 1, 2009. Individual task schedules aze noted under each task. I have <br /> made some alterations to the tasks based on my interpretation of the discussions with resource <br /> representatives from ODOT during our phone conversation on Monday November 24, 2008. <br /> 1.0 TASKS AND DELIVERABLES <br /> Task 1- Coordination, Accumulation, and Review of Information <br /> Environmental Solutions LLC shall obtain and review information necessary to the understanding of the <br /> project and collect information and data necessary for completion of the JPA, as outlined in OAR 141- <br /> 085-0025 as necessary. Environmental Solutions LLC shall prepaze a draft JPA for review by the City of <br /> Eugene (City). This will include coordination with the City to obtain the latest drawings, details, <br /> descriptions, and specifications to support development of the 7PA, and include informationfrom Task 5 <br /> of the Wetlands and Waters Reporting and Permitting proposal. Environmental Solutions LLC shall <br /> prepaze drawings, maps, and figures for the JPA, and coordinate with the City to ensure that the features <br /> and impacts aze correctly identiSed in the JPA. Environmental Solutions LLC shall document the effect <br /> of project on ESA species, based on information from the Biological Assessment or Biological Baseline <br /> Report. Based on information provided by the cultural resources assessment, Environmental Solutions . <br /> LLC shall document the effect of the project on historic and cultural resources. Environmental Solutions <br /> LLC shall make recommendations regazding next steps needed to complete the application including a <br /> potential site visit. Environmental Solutions LLC shall also identify the appropriate Corps and DSL <br /> permit that will authorize the activity and provide rationale for the choice, as well as other resource plans <br /> required (ie. Restoration Plan, Mitigation Plan, Stormwater Management Plan, Sediment and Erosion <br /> Control Plan), although Environmental Solutions LLC will not prepare the Stormwater Management Plan, <br /> Sediment and Erosion Control Plan if required for the project. <br /> Deliverables: Draft JPA, completed JPA checklist filled out and annotated to reference the location of <br /> information in the draft JPA, memo of recommendations for next steps and additional information needed <br /> to complete the JPA. The City of Eugene will provide the Stomiwater Management Plan, Sediment and <br /> Erosion Control Plan.
