'i <br /> In the event th permittee contests responsibility for the damage to a City facility discov- <br /> fired within the permittee's work zone, the permittee shall still be held responsible for <br /> effecting imme late repairs. The permittee should then be directed to submit a billing to <br /> the Maintenan a Planning Manager showing the exact time and materials expended to <br /> effect the repa rs. Also, the permittee should include, in narrative form, an explanation as <br /> tp the cause o the damage, the reasons the permittee should not be held responsible, <br /> i~nd justificatio for the billing. <br /> j Damag to Sanitary Sewers - If the damaged facility is a sanitary sewer line ten <br /> (10) inc es in diameter or less, the repairs shall be performed by a contractor, <br /> plumber utility crew, or others qualified to perform such work. The work shall be <br /> perform din accordance with City Standards and inspected by the Utility Inspector <br /> prior to ackfill. <br /> All "taps into City sanitary sewer mains shall be performed by the City. <br /> When th damaged facility is a sanitary sewer line larger than ten (10) inches in <br /> diamete ,the Wastewater Collections Crew Supervisor shall be immediately <br /> notified. The Crew Supervisor shall have the option of providing repairs consul- <br /> tation to the permittee responsible for the damage, directing the repair performed <br /> <br /> ' I by the p rson(s) responsible for the damage, or having the repair performed by City <br /> crews at the expense of the permittee responsible for the damage. The Utility <br /> Inspect shall submit a damage and inspection report to the Maintenance Planning <br /> Manage who shall review and approve a billing submittal to the permittee responsi- <br /> <br /> 9 ! ble for r pair costs incurred by the City. <br /> i Damag to Stormwater System Components - If the damaged facility is a storm <br /> sewer li e, the repairs shall be performed by a contractor, plumber, utility crew, or <br /> others q alified to perform such work. The work shall be performed in accordance <br /> with City Standards and inspected by the Utility Inspector prior to backfill. Repairs <br /> 'i involvin taps to a storm sewer may be performed by qualified contractors or <br /> personn I working for the permittee. <br /> Catch b sins, inlets, culverts, roadside ditches, and drainage channels damaged by <br /> the work of the permittee shall be restored to as good a condition as existed prior to <br /> permit w rk. Catch basins shall be cleaned of material or debris resulting from the <br /> work. Di ches and channels shall be restored to original shape with banks left in <br /> stable c ndition. Rock used to restore ditch or channel slopes shall be a minimum <br /> 3" and c mpacted, or as otherwise specified by the Utility Inspector. Damaged <br /> culverts hall be repaired and cleaned of construction debris. <br /> Damage to Traffic and Street Light Facilities - If a street light, sign, signal wire, <br /> detector oop, or related facility is damaged, the Utility Inspector and City Transpor- <br /> tation M intenance (682-5218) shall be immediately notified. It is important to note <br /> that if a etector loop is cut, the entire run must be replaced.
