The request for an annual permit will be reviewed and approved, approved with condi- <br /> tions, or denied. Annual permits are subject to revocation if a facility owner fails to comply <br /> with the standards established by the City or conditions of the permit. <br /> Permit fees for annual permits include a processing component and a "per activity" <br /> component. The permit processing component is payable upon approval of the annual <br /> permit. The "per activity" component is calculated monthly or periodically, based on the <br /> number of right-of--way cuts performed under the annual permit, and billed to the permit- <br /> tee. <br /> 2.C REVOCABLE PERMITS FOR PRIVATE FACILITIES <br /> A revocable permit may be issued to a permittee for any use of the public way which is not <br /> administered by an existing franchise or license. As with all right-of--way permits, the City <br /> reserves the right to "revoke" or cancel the permittee's permission to use the public way at <br /> a later date. This type of permit is typically used for placement of a private facility that <br /> serves an individual property, site, or business and is not placed for the provision of <br /> services to others. Detailed questions concerning revocable permits should be addressed <br /> to the Engineering Division personnel. <br /> Unless otherwise specified in a license or franchise agreement, a revocable permit shall <br /> be obtained prior to the placement of any private facility upon, over or under the public <br /> way. The permittee shall be required to pay all permit fees and user fees as established. <br /> Issuance of a revocable permit does not relieve the permittee from obtaining any <br /> other permits which may be required. Examples of other permits which may be <br /> required are building, right-of.--way cut, tree removal, and sewer connection. Any revoca- <br /> ble permit which entails excavation within the public way shall also require aright-of--way <br /> cut permit. <br /> Revocable permits issued which involve the placement of underground facilities shall <br /> include the requirement for the permittee to subscribe to the Oregon Utility Notification <br /> Center and to respond to requests from underground excavators for the field location of <br /> the permittee's facilities. Failure to maintain this subscription may result in the permit <br /> being revoked. <br /> Revocable permits shall be subject to all standards for temporary traffic control, construc- <br /> tion practices, and right-of-way restoration included herein. <br /> 2.C.1 Application <br /> <br /> Application to use the public way under a revocable permit may be made to the staff of the <br /> City Engineer at the Permit and Information Center at 99 West 10th Avenue. <br /> Utility & R/W Permit Policy 8 06/01/97 <br /> <br />
