When t e gutter bar portion of the curb and gutter is removed along an asphalt <br /> street, t least 24 inches of asphalt concrete shall be removed to allow sufficient <br /> room fo placement of the forms for the new face of gutter and proper compaction <br /> of asph It restoration. An alternate to removal of adjacent asphalt shall be to saw <br /> cut the sphalt at the face of the gutter bar, using the edge of asphalt as the form <br /> for con rete replacement. When the curb is removed but the gutter bar is left in <br /> place, oxy shall be used on the back of the gutter to bond the existing P.C.C. to <br /> the ne P.C.C. being placed (i.e. at driveways, handicapped access ramps, etc.). <br /> 4. Any tra is control devices (signing, striping, pavement markers, traffic signal loops, <br /> etc.) da aged or removed during the work allowed by the permit shall be replaced <br /> by the ermittee. <br /> 5. Bicycle wheelchair ramps shall be constructed whenever a curb or sidewalk is <br /> constru ted, improved or replaced at any point in a block which gives reasonable <br /> access o a crosswalk and where no ramp currently exists. (ORS 447.310) <br /> 6. The pe mittee shall notify the Utility Inspector at least one full business day in <br /> advanc of the time any permanent surface restoration is to be made by the permit- <br /> <br /> ' tee's o n staff or that of contractors working for the permittee: . <br /> 7. The ele anon of finish grade for underground vaults shall be at least 18" below the <br /> finish g ade of the surface A.C. or P.C.C. <br /> 8. Above round facilities shall not be placed in a manner which would cause a vision <br /> obstru ion as defined by Eugene Code Section 6.010 Q). <br /> 19. When ewer lateral markers in new developments (typically 2"x 4" wood posts) are <br /> damag d, they shall be replaced by the permittee or the permittee's contractor. <br /> This re lacement shall require excavation to uncover the sewer lateral and ensure <br /> the cor ect location for the replacement marker. <br /> 0. Catch asins or storm inlets abutting the work area shall be inspected by the <br /> permitt e or their contractor at the conclusion of work and cleaned of construction <br /> related debris as necessary. <br /> Where the Ut lity inspector requires a bicyclelwheelchair ramp to be constructed under <br /> ORS 447.310 guidelines and where the existing sidewalk and/or bicycle/wheelchair ramp <br /> has extensiv existing damage, the permittee may request the City to participate in the <br /> expense of c nstructing the new ramp. The permittee must request the Utility Inspector <br /> to meet with t e permittee at the location of the proposed work to evaluate the existing City <br /> sidewalk/ram prior to the commencement of any work.
