2. Identification of the types of activities and scope of work (length and diameter sizes <br /> of service lines, etc.) requested to be allowed under the annual permit. <br /> 3. A monthly report of all blanket permit activities and right-of--way cuts will be <br /> submitted, in an agreed-upon format, of each service installation or repair <br /> performed by the permittee's staff or by contractors working for the permittee. <br /> 4. The name and telephone number of a specific "contact person" who will be <br /> responsible for responding to City concerns regarding the permittee's field activities. <br /> The request for an annual permit will be reviewed and approved, approved with conditions, <br /> or denied. Annual permits are subject to revocation if a facility owner fails to comply with <br /> the standards established by the City or conditions of the permit. <br /> Permit fees for annual permits include a processing component and a "per activity" <br /> component. The permit processing component is payable upon approval of the annual <br /> permit. The "per activity" component is calculated monthly or periodically, based on the <br /> number of activities performed under the annual permit for which a City response (e.g., site <br /> review, inspection) was performed, and billed to the permittee. <br /> 2.C REVOCABLE PERMITS FOR PRIVATE FACILITIES <br /> A revocable permit may be issued to a permittee for any use of the public way which is not , <br /> administered by an existing franchise or license. As with all right-of--way permits, the City - <br /> reserves the right to "revoke." or cancel the permittee's permission to use the public way <br /> at a later date. This type of permit is typically used for placement of a private'facility that <br /> serves an individual property, site, or business and is not placed for the provision of <br /> services to others. Detailed questions concerning revocable permits should be addressed <br /> to the Engineering Division personnel. <br /> Unless otherwise specified in a license or franchise agreement, a revocable permit shall <br /> be obtained prior to the placement of any private facility upon, over or under the public <br /> way. The permittee shall be required to pay all permit fees and user fees as established. <br /> Issuance of a revocable permit does not relieve the permittee from obtaining any <br /> other permits which may be required. Examples of other permits which may be required <br /> are building, right-of--way cut, tree removal, and sewer connection. Any revocable permit <br /> which entails excavation within the public way shall also require aright-of-way cut permi#. <br /> Revocable permits issued which involve the placement of underground facilities shall <br /> inform the permittee of the requirement for the permittee to subscribe to the Oregon Utility <br /> Notification Center and to respond to requests from underground excavators for the field <br /> location of the permittee's facilities.
