All other applicants for <br /> permits to per orm work in the public way shall make their applications through Public Works staff <br /> at the Permit nd Information Center. Applications shall be made on a standard form provided by <br /> the City. All r levant portions of application form and standard attachments must be completed prior <br /> to submittal o the City. Incomplete applications ti-ill not be processed and will be returned to <br /> applicant for ompletion and re-submittal. <br /> Application f r a permit to perform work in the right-of--way must be made by a representative of <br /> the owner or perator of the facility being installed or upon which maintenance is being performed. <br /> Persons or c mpanies working under contract to the facility owner or operator are not considered <br /> representativ s of the facility owner and as such will not be allowed to apply for or be granted a <br /> permit. <br /> Unless a spec al exception is granted, applications for permits to perform work in the right-of--way <br /> to perform ex avations upon improved streets shall not be granted where the pavement is less than <br /> five years old.
