depending up n the complexity of the individual permit situation. <br /> A copy of all ermits granted shall be forwarded to the Inspector in Maintenance Planning to allow <br /> inspection of he right-of--way restoration and temporary traffic control measures used. <br /> 2.D Stree and Right-of--Way Work Permits <br /> 2.D1 A li ation <br /> For EWEB, f anchised utilities and licensees, application to perform work within the public way <br /> shall be made to the Maintenance Division at 1820 Roosevelt Boulevard. All other applicants for <br /> permits to per orm work in the public way shall make their applications through Public Works staff <br /> at the Permit nd Information Center. Applications shall be made on a standard form provided by <br /> the City. All r levant portions of application form and standard attachments must be completed prior <br /> to submittal o the City. Incomplete applications ti-ill not be processed and will be returned to <br /> applicant for ompletion and re-submittal. <br /> Application f r a permit to perform work in the right-of--way must be made by a representative of <br /> the owner or perator of the facility being installed or upon which maintenance is being performed. <br /> Persons or c mpanies working under contract to the facility owner or operator are not considered <br /> representativ s of the facility owner and as such will not be allowed to apply for or be granted a <br /> permit. <br /> Unless a spec al exception is granted, applications for permits to perform work in the right-of--way <br /> to perform ex avations upon improved streets shall not be granted where the pavement is less than <br /> five years old. EC 7.295 (2) <br /> Z.D.2 Plans and Drawin s <br /> Applications for a permit shall be accompanied by such drawings, surveys or plans as deemed <br /> necessary by he City Engineer. EC 7.305 <br /> Generally, th permittee shall be required to supply two sets of complete and accurate plans and/or <br /> drawings dep cting the proposed facility installation.
