PERSONAL SERVICES CONTRACT <br /> Architect and Engineer <br /> BETWEEN: The City of Eugene, an Oregon <br /> Municipal Corporation <br /> AND: Pavement Services Inc. <br /> Contract No.: 2007-00216 <br /> This contract is being established per the terms and conditions of Personal <br /> Services Master Contract master contract #2003-00216 dated November 21, <br /> 2002, between the City of Eugene (City) and Pavement Services Inc. <br /> (Contractor). <br /> City and Contractor mutually agree to the attached scope of work as outlined <br /> herein and the contract referenced above as follows: <br /> Provide pavement services design recommendations for the Chad Drive <br /> <br /> ~rl Extension (Exhibit A) and North Game Farm Road (Exhibit B) <br /> Improvement projects.
