<br /> i <br /> Amendment to Scope of Work - Attachment A CH2MHILL <br /> Polk Street Outfall Water Quality Improvements, <br /> Project # 3984 -Scope Amendment <br /> PREPARED FOR: Terry Colvin/City of Eugene <br /> PREPARED BY; Mark Bransom/CH2M HILL <br /> DATE: June 16, 2006 <br /> DESCRIPTION <br /> CH2M HILL has completed compiling and reviewing existing information, and developing <br /> alternatives for near-term water quality improvements in the Polk Street Basin. One of these <br /> alternatives, the vault retro-fit project, was selected to proceed to design phases. Task 3 of <br /> the initial scope of work included development of 50% and final designs and construction <br /> documents for the selected projects. A 50% design of the vault retrofit project was <br /> completed and submitted. This has been reviewed by the City, with recommendations <br /> made for incorporation in a final design. <br /> The vault retrofit design will include several elements not included in the original scope. <br /> Particularly, these include geotechnical design, structural design, site civil design of access <br /> routes and maintenance facilities, additional sanitary sewer pipe connections, and <br /> mechanical design. In support of the City's efforts to reach decisions about the project, <br /> CH2M HILL has provided additional services during the 50% design and subsequent <br /> response to comments. This amendment intends to capture work done to date that has been <br /> out of the original scope of work, as well as tasks needed to complete preparation of design <br /> and construction documents for the vault retrofit project. In addition, some tasks from the <br /> original scope of work are no longer needed and have been deleted as a means of cost- <br /> effectively delivering the project as currently envisioned. <br /> TASKS <br /> Task 1.1-Delete summary of data review deliverable <br /> Task 2.1-Delete finalized alternatives analysis memorandum deliverable <br /> Task 3 -Delete Final (100%) Submittal and replace with Tasks 7.1-7.5, described below <br /> Task 3 -Delete consultant on-call services during construction and replace with Task 7.6 <br /> below <br /> Task 4 -Delete basin management strategies deliverable and brainstorming meeting <br /> Task 7.
