Section 00401--Page 1 <br /> CERTIFICATE B <br /> CERTIFICATION OF NONSEGREGATED FACILITIES <br /> Applicable to government contracts and subcontracts exceeding $10,000 and not exempt from <br /> the provisions of the Equal Opportunity Clause as provided by Executive Order No. 11246 and <br /> regulations promulgated thereunder. <br /> The undersigned hereby certifies that he does not and will not maintain any facilities for his <br /> employees in a segregated manner or permit his employees to pertorm their services at any <br /> location under his control where segregated facilities are maintained. The undersigned <br /> understands that "segregated facilities" means those which are in fact segregated on the basis <br /> of race, religion, color, or national origin, or because of habit, local custom, or otherwise. The <br /> undersigned further understands that "faciities" indudes waiting rooms, work areas, restaurants <br /> and other eating areas, time clocks, rest rooms, washrooms, locker rooms and other storage or <br /> dressing areas, parking lots, drinking fountains, recreation or errtertaining areas, transportation, <br /> and housing facilities provided for employees. <br /> The undersigned understands and agrees that a breach of this assurance is a violation of the <br /> Equal Opportunity Clause required by Executive Order No. 11246 of September 24, 1965. <br /> The undersigned further obligates him/herself to obtain and retain a substantially similar <br /> assurance for each subcontract or for specific time periods (i.e., quarterly, semi-annually, or <br /> annually) from proposed subcontractors prior to the award for any subcontract exceeding <br /> $10,000 and riot otherwise exempt from the requirements of the Equal Opportunity Clause as <br /> provided by Executive Order No. 11246 and regulations promulgated thereunder. <br /> The undersigned further acknowledges an obligation of contractors, subcontractors, and <br /> suppliers having 50 or more employees and an agreement, purchase order, or contract in <br /> excess of $50,000 to report annually on Standard Form 100 (EEO-1) and to develop and <br /> maintain for each of its establishments a written Affirmative Action Program meeting the <br /> requirements prescribed by the regulations published in Title 41, Chapter 60~of the Code of <br /> Federal Regulations. <br /> This certification shall be valid for a period of one year from the date of signing. <br /> NOTE: The penalty for making false statements is prescribed in 18 U.S.C.¶1001. <br /> Contractor Name ~ ~ ~ / Subcontractor Name <br /> Date Signature Title <br /> <br /> RFB No. 2003200138 Standard Bid FomdPubiic Works Improvement <br /> Revised Fetxuary 1997 <br /> <br />
